Transition Resources for Ages 14-21

Welcome to the Eau Claire Area School District Transition Resource site.  Our district has two full time Transition Coordinators.  Our goal is to provide support for students with disabilities (ages 14-21) and their families with education, resources, technical assistance, and community connections. Transition planning is unique to each student based on their post high school goals.   
 
Below are links to resources utilized in our district to facilitate transition for a student from ECASD to life beyond high school.  These are not all-inclusive resources but serve as guides to start the transition process.
