Immunizations

We would like to extend a welcome to elementary school to you and your child from the Eau Claire Area School District, and inform you of immunization requirements for kindergartners.

Wisconsin State Law requires that all immunizations be up-to-date within 30 days of entering the school system. New students will be asked to provide a record of immunizations at registration; all others are requested to notify the school when your child receives additional immunizations. Please take the opportunity to take care of this prior to the start of school.

Immunization Requirements for Incoming Kindergarteners:

     • DTap, dose 4

     • Hepatitis B, dose 3

     • MMR, dose 2

     • Polio, dose 4

     • Varicella, dose 2
