Welcome 7th and 8th graders to another exciting season of Polar athletics.  There are several very important items that you need to be aware of and understand in order to partake.  The following eligibility requirements are necessary of all athletes.

 **The signatures on your physical card also indicate that the parent/guardian and athlete have seen, read, understand, and will comply with the provisions of the Eau Claire School District Middle School Athletic Code.  If you plan to play any sport at Northstar, be sure to read the Athletic Code, and share it with your parents/guardians.  Middle School Athletic Code

You must have a current green Physical Card on file with the Northstar Athletic Director’s Office on or before the first day of practice. Current physicals on file, dated after April 1, 2020, will allow athletic eligibility for the entire 2021-22 school year. This card must have the date of your physical exam, a signature of the health provider who preformed the exam, and the signature of your parent/guardian.

Parent/Guardian and athlete must review and sign the Parent & Athlete Agreement and Concussion Acknowledgement form located on page 2 of the Wisconsin fact sheet for parents.

Fee Collection Form
Required
 
Fee collection form and payment must be turned in prior to the athlete’s first contest. You may pay individually for each season or, for your convenience,  you may pay for all seasons at one time
 
Students who receive no discount on meals will pay $40 per sport.
 
Students who qualify for reduced priced meals will pay $20 per sport
 
Students who qualify for free meals will not pay a fee.
 
  Emergency Transportation and Treatment Form
Required
 
The emergency transportation form must be filled out and returned prior to the first athletic contest. This form requires insurance information, athlete signature, and parent/guardian signatures.

  ImPACT Testing Permission
 Optional but strongly suggested
 
ImPACT (Immediate Post-Concussion Assessment and Cognitive Testing) is the most-widely used and most scientifically validated computerized concussion evaluation system. ImPACT provides trained clinicians with neurocognitive assessment tools and services that have been medically accepted as state-of-the-art best practices as part of determining safe return to play decisions. Athletes have the option to complete a “video game” like computerized exam early in the season.  This exam provides a baseline to the athletes brain which is invaluable to medical professionals should a concussion occur.

Quarter 1: August 30- October 28
Quarter 2: November 1- January 15
Quarter 3: January 20- March 17
Quarter 4: March 28- May 26
  Practices All sports except volleyball and basketball:
M, T, Th, F 3:00–4:30p.m.
Volleyball & basketball practices:
7th grade 3:00-4:30p.m. 
8th grade and 4:30-6:00 pm.
